Aphasia/Language
- The Arizona Semantic Test was developed by the Aphasia Research Project at the University Of Arizona, it is a free, printable nonverbal test of semantic knowledge requiring pointing responses.
- The Brisbane Evidence-Based Language Test, is a free test for SLPs to use in acute stroke populations. It includes subtests for the following areas: perceptual, auditory comprehension, verbal expression, reading, and writing.
- The Quick Aphasia Battery (QAB) is provided free online by Vanderbilt University. Feel free to download the paper, score sheets, and stimulus printables, all free of charge.
- The Center for Outcome Measurement In Brain Injury (COMBI) has provided the Mississippi Aphasia Screen Test ( also known as the MAST) including the rating form, written commands and a link to the verbal fluency subtest stimulus for free.
- The Multimodal Communication Screening Test for Aphasia (MCST-A) is a criterion referenced assessment in the AAC Assessment Battery of Aphasia by K. Garrett and J. Lasker, available on the University of Nebraska-Lincoln website. It is used to assess whether people with severe aphasia can use alternative or augmentative modalities via alternative pictorial symbols. Also included are an Aphasia needs assessment, categories of communicators checklist, and a scanning/visual field/print size/attention screening task. Go check it out!
- The Philadelphia Naming Test is a free 175-item picture naming test developed in the Language and Aphasia Lab of MRRI (Moss Rehabilitation Research Institute).
- The Communication Confidence Rating Scale For Aphasia by Babbit & Cherney, 2010, is a questionnaire that can be used to formulate functional goals for patients or utilized as a pre- and -post measure.
- The Communicative Effectiveness Index is a 16 item functional communication measure of adult aphasia.
Cognition/Dementia
- Addenbrooke's Cognitive Examination (ACE-III) is available from the University Of Sydney's Brain and mind Centre. The examination is free and provided in a multitude of languages, including English and English- USA versions. It is also available in a mini-version as well as on the iPad.
- The Cambridge Semantic Memory Battery is a free, downloadable test that assesses semantic memory.
- The Cambridge Behavioural Inventory (CBI) is a free questionnaire given to caregivers that examines patient's behaviors related to cognition.
- The Driving Self-Assessment by B-Cat is a questionnaire to identify safety issues associated with driving.
- The Short Blessed Test (SBT), which is also referred to as the Six Item Cognitive Impairment Test (6CIT) is a very short cognitive screener, and free to print!
- VAMC Saint Louis University Mental Status Examinations a commonly used screen to identify the presence of cognitive deficits, and to identify changes in cognition over time. The original version: SLUMS and the large print versions offered in either English and Spanish versions are linked HERE.
- Montreal Cognitive Assessment (MOCA), version 7.1, is a cognitive screen.
- The Multifactorial Memory Questionnaire (MMQ) is a self-report to use for assessing meta memory in middle aged and older adults. Three scales measure satisfaction with memory functioning, self-evaluation of memory ability, and self-reported use of memory strategies.
- Frontotemporal Dementia Rating Scale (FRS) is a free severity rating tool to be utilized with the main carer of the individual with FTD and includes a brief interview.
- Frontier Executive Screen (FES) is a free brief battery to help with differentiating Fronto-Temporal Dementia and Alzheimer's Dementia.
- The Repeat & Point Test helps with differential diagnosis of Semantic Dementia from Progressive Nonfluent Aphasic patients.
- The Self-Administered Gerocognitive Examination (SAGE) requires a pen and paper only and is designed to detect cognitive, memory for thinking impairments.

Swallowing/Dysphagia

- The Reflux Symptom Index is a very short questionnaire to help determine if significant reflux disease is present.

Voice
- The Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V) was developed by the ASHA Group 3 as a clinical tool to describe the severity of attributes of a voice problem. Attributes including overall severity, roughness, breathiness, stain, pitch, and loudness of voice are rated by the clinician.
- The Voice Handicap Index (VHI) measures the influence of voice problems on a patient's quality of life and is a useful assessment tool for voice disorders.
Dysarthria
- The NewCastle Dysarthria Assessment Tool (N-DAT) utilizes a stopwatch, mirror, voice recorder, the cookie theft picture and/or the caterpillar passage.
- Tikofsky's 50-word Intelligibility Test requires no special materials. and helps to quantify the degree of dysarthria impairment based on intelligibility. It is composed to 50 words and is scored by counting the number of words correctly heard and that number is then multiplied by 2 to give a percent intelligibility rating.
